Accreditation Council of Graduate Medical Education (ACGME) Milestones were introduced in 2014 and developed for use in assessing residents while they participate in ACGME programs. The ACGME also uses board certification exam passage rates “as an indicator of [residency] program quality.” The objective of this study is to “investigate the validity of the milestones by demonstrating its association with performance in the [American Board of Physical Medicine and Rehabilitation] ABPMR certifying examinations.” 233 physical medicine and rehabilitation residents who took the ABPMR initial certification examinations (Part I and Part II) were initially identified, and ultimately 207 residents met the inclusion criteria. Results showed that Part I examination scores were strongly correlated with medical knowledge (MK) milestones “but not with milestones for other competencies” including professionalism, patient care, practice-based learning, interpersonal and communications skills, and systems-based practice. Conversely, “[m]ilestone ratings did not correlate with Part II scores.” The authors conclude that the results of this study could provide support for those residents in need of remediation and “help address potential competency gaps” in the specialty of physical medicine and rehabilitation, as well as improve certification examination performance. Coming Together to Save Babies: Our Institution's Quality Improvement Collaborative to Improve Infant Safe Sleep Practices Infant sleep-related deaths are one of the leading causes of infant mortality in Ohio’s Franklin County. The objective of this quality improvement (QI) study was to reduce Franklin County’s “sleep-related death rate by improving infant safe sleep practices,” screening, and education in hospitals, clinics, and emergency rooms. The study period was 2015-2019. A four-team multidisciplinary quality improvement collaborative was created from Nationwide Children’s Hospital’s inpatient services, neonatal intensive care units, the emergency department, and the primary care network. The teams used QI strategies such as key driver diagrams and plan-do-study-act (PDSA) cycles. Physicians who participated in this QI project were able to obtain Maintenance of Certification (MOC) part IV credit. (PDSA cycles are required for a physician to obtain MOC part IV credit for a QI activity.) While the collaborative results showed significant improvements in infant safe sleep practice behavioral changes, screening of parents, and educational processes at the institutional level, as well as a “decrease in sleep-related deaths” in the primary care network, the goal of reducing sleep-related infant mortality in Franklin County was not met. The authors conclude that while their QI project did result in improved practices and a reduction of infant mortality in their primary care network, they will need to engage the local community in order to achieve a reduction in Franklin County infant mortality rates. Nationwide Children’s Hospital is an ABMS Portfolio Program sponsor. Multi-component Cancer Prevention Awareness Program to Improve Adolescent HPV Vaccine Uptake The objective of this study was to evaluate an educational program on human papillomavirus (HPV) vaccinations. Eight pediatric and three family medicine practices located in suburban, rural, and urban areas serving 31,000 adolescents were recruited and participated in this quality improvement (QI) project between 2017-2018. The project duration was one year. Combined, 58 health care providers participated in a project survey and physicians were able to receive Maintenance of Certification (MOC) part IV credit. Vaccination rates were determined at baseline and re-evaluated after a year. Educational materials and sessions were disseminated and implemented. Results showed that “all of the recruited practices demonstrated increases in HPV vaccine series initiation rates,” and although completion rates increased for the 13-18 year old age range, only “7/11 practices increased their HPV vaccine series completion rates” for 11-12 year olds. The authors conclude that while improvements were shown as a result of the year-long quality improvement project, there is still significant variability between pediatrics and family medicine, genders receiving the vaccine, and individual providers. Continuous Improvement in Patient Safety and Quality in Neurological Surgery: The American Board of Neurological Surgery in the Past, Present, and Future This article provides a brief overview of the history of the American Board of Neurological Surgery (ABNS), a member board of the American Board of Medical Specialties (ABMS), from its founding in 1940 to the present day. The paper describes the ABNS Board of Director make-up, updated requirements for board eligibility, certification examination origin and evolution (both written and oral), developments in focused practice recognition, and the ABNS’ new adaptive learning tool. The authors conclude by stating that recent updates to their program will allow continued improvements “to better serve the field of neurological surgery and the cause of public health.” How Should Board Certification Evolve? This review article discusses ways that physician specialty board certification must evolve with the changes in society and health care. The a...
